9.《外国人体格检查表》复印件（原件自行保存，此表格由中国卫生检疫部门统一印制，须英文填写）

申请人应严格按照《外国人体格检查表》中要求的项目进行检查。缺项、未贴有本人照片或照片上未盖骑缝章、无医师和医院签字盖章的《外国人体格检查表》无效，检查结果有效期为6个月。

10.无犯罪记录证明

申请人须提交由所在地公安机关出具的有效期内的无犯罪记录证明，通常应为提交申请之日前6个月以内的证明文件。

Prospectivediplomarecipientsmustsubmitofficialproofofstudentstatusintheircurrentschool.DocumentsinlanguagesotherthanChineseorEnglishmustincludeattachmentswithnotarizedChineseorEnglishtranslations;

4.Academictranscripts(fromtheundergraduateprogramonwards)

Applicantsshallprovidecopiesofacademicrecordsfromthefirsttothelastsemesterduringtheundergraduate,master’s(ifany),anddoctoralprograms(ifany).Thesetranscriptsshallbeissuedandsealedbytheattendeduniversity’sacademicaffairsoffice,graduateschool,orstudentmanagementdepartment.TheycanbeinChineseorEnglish,andmustbeattachedwithnotarizedEnglishtranslationsiftheyareinforeignlanguagesotherthanEnglish;

9.AphotocopyoftheForeignerPhysicalExaminationFormcompletedinEnglish(theapplicantshallkeeptheoriginalcopy)

TheformisissuedbytheChinesequarantineauthorityandcanbeobtainedfromthedispatchingauthority.ThephysicalexaminationsmustcoveralltheitemslistedintheForeignerPhysicalExaminationForm.Incompleterecordsorthosewithouttheattendingphysician’ssignature,theofficialstampofthehospital,orastampedphotographoftheapplicantsareinvalid.Pleaseselecttheappropriatetimetotakeaphysicalexamination,astheresultisvalidforonly6months.

10.Non-criminalrecordreport

TheapplicantshallsubmitavalidcertificateofNon-CriminalRecordissuedbythelocalpublicsecurityauthority,usuallyissuedwithin6monthspriortothesubmissiondateoftheapplication.

1.申请人在“留学中国”网站http://www.campuschina.org申请，点击“中国政府奖学金留学管理信息系统”图标进入申请系统；

2.填写申请人“个人资料”，留学项目种类选择“B类”，南京医科大学机构编号为10312；

Inputyourpersonaldetails,andselectProgramCategoryTypeB.TheAgencyNumberofNanjingMedicalUniversityis10312.

3.录入“语言能力及学习计划”并上传“补充材料”，直至点击【提交】完成申请。

Moveontothesectionof“LanguageProficiencyandStudyPlan”anduploadthe“SupportingDocuments”asrequested,thenclick“Submit”tocompletetheapplicationsubmission.

第二步：考试安排StepTwo:Interviews

申请人资料审核通过后，学校会通过邮件通知申请人进行面试。

第三步：录取与入学StepThree:Admission

申请人可在“中国政府奖学金留学管理信息系统”查询录取结果。被录取后，学生须严格按照学校规定的时间报到注册。报到时将核验最高学历证书及成绩单原件、《外国人体格检查表》、无犯罪证明、语言水平证明原件等申请材。

ApplicantscanchecktheadmissionresultsintheChineseGovernmentScholarshipStudyAbroadManagementInformationSystem.Uponreceivingtheadmissionnotice,applicantshallregisterattheNanjingMedicalUniversitywithinstipulatedtimewithalloriginalapplicationmaterials.

*境外学生入校报到事宜将根据中国入境政策另行通知。

*TheregistrationprocedurefortheoverseasstudentswillbenotifiedseparatelyinaccordancewithChina'sentrypolicy.
